I was doing a firmware upgrade on several test phones when I discovered one of the phones that did not upgrade does not have a TFTP Server 1 IP address.
I tried to assign it on the phone itself but I'm unable to. When I click edit it says "the key is not active".
I've tried resetting the phone several times but to no avail. I'm about to remove/re-add the phone from CM but thought I'd ask you experts first if there's a fix other than removing/re-adding the phone in CM.

Are TFTP options configured on your DHCP server?
Use option 150 and 66.

Yes, DHCP is setup with couple of TFTP servers.
I just discovered that this phone is not in the voice vlan. I went ahead and changed the vlan settings and voila!!
