Hi,
I have a CME version 8.6 .
I have configured MOH and it is working fine for PSTN calls or external calls.
But it is not working for internal IP Phones.
Please suggest.

Also, make sure the Loopback and the voice vlan interface having 'ip pim den' enabled.

Have you configured the multicasting for the local MOH

Try to change the port to 16384.

Is this a Live Feed MOH? If yes, here is the example for Live Feed MOH:

A few things:
Your configuration looks good. I am wondering what the IP address of the loopback interface is? It is ideal to configure the ip addresses in this fashion.
The fact that it is working on the PSTN tells me it is related to one of two things.
Mulitcast routing on your switch:
Make sure multicast routing is enabled on the switch, if the IP Phones are on a different subnet.
or,
A problem on your platorm, that may require you to call TAC, what version of IOS are you running?

which all hops should be there after the route keyword.
Whether 224.91.22.5 can be used for multicasting?

As there are no MoH groups in CME, MoH is not used for internal calls.
