We have noticed in our CUCM version 11.5.1.12900-21 that remote destinations stop working if the users does not answer several calls in a row. We have to go in and reset the destination by unchecking the line association and checking again. This will work until the user does not answer several calls.
This is not an expected behavior, so there's no quick answer or an easy fix (if this is what you were hoping for). The issue needs to be investigated, so I suggest opening a TAC case.
