When I try to use the Join softkey to Join two participants together, I receive a "Cannot Complete Conference" error. However, conferencing itself seems to be working fine (taken from a 3 person test conference):

I've tried changing from hardware to software conferencing and tried fiddeling with a few other things to no avail.
The user guide doesn't even document Join for CME. I see the feature documented in the admin guides, however, so I assume it should be available for use. I tried using Join with one call on hold and one call active and I tried using Join with two calls on hold. Both tests give the same result.

You need to add a SELECT key in the
ephone template for softkeys hold.Then when you have the 2 calls on hold press the select key for both lines, you will see a check box.
after both are selected then press join.

Exactly how are you trying to use the join softkey???
What are you doing/pressing on the phone??
•Join—Joins an established call to a conference. You must first press Select to choose each connected call that you want to join in a conference, then press Join to join the selected calls to the conference.

From my phone (Phone A), I call Phone B. I place that call on hold. I then place a new call to Phone C. I then try to join Phone B into the call.
So, I have a single line key on my phone (octo-line) with an active call and an on-hold call. At this point, I have an active Join key. If I arrow up to the on-hold call, then press Join, I still get the conference error (is this what it means by pressing Select?).
I ran this scenario in IP Communicator so I could get a screenshot.

Have you tried with a dual-line DN ?

Just tested with dual-line DN as the originating caller (the one trying to use join). Still get the exact same thing.

Thank you. For some reason, I totally didn't get that from the documentation.
If only features would work the way I think they should rather than the way Cisco thinks they should ;-)
