Voicemails get sent to 2 extensions

When a user calls ext 1000 and leaves a voicemail, the message is also placed in ext 1001's voicemail box.

No alternates are set in either users profile in Cisco Unity.

Any reason why this would be occurring?

Maybe Relay is set up in Message Actions?

Check the distribution list; there must be one causing the voicemail to be sent to the 1001 mailbox as well.
