How can you determine if your MOH is streaming from the router flash?
Does it only stream from flash in SRST Mode

Take a look at the following document. There are some commands you can use to verify MOH like show ccm-manager moh. You can use Multicast MOH from the gateway in SRST mode as well as during normal operations.

It definitely did. If there is an active session on the gateway, does that mean it is streaming from flash?
example:
#sh ccm-manager music-on-hold
Current active multicast sessions : 1
Multicast RTP port Packets Call Codec Incoming
Address number in/out id Interface
===============================================================239.1.1.1 16410 1620/1620 161373 g711ulaw Gi0/0

Yet it does. The router is streaming MoH in your case!
