I am receiving the following error when access Fast Dials from 7960 phone:
Error: -7ffbc17c: Accessing an empty list.
Note: this is not an invalid password error.
Any ideas??

What CCM version are you running? There are some bugs for old versions about this.
Cannot add Fast Dials when CCMUser page points to the Subscriber Server in a Cluster
running 3.2(2c) spH or spG.
Symptoms:

Adding the first Fast Dial leads to the following error
Error While Loading Page
Accessing an empty list (-2147205500)
Adding Consequtive Fast Dials leads to the following errors
Error While Loading Page
Unable to find Fast Dials entry (UserName3-fd1049392796291) (1)
WorkAround:

1) Try adding the Fast Dial once more after you get the error and it gets updated.
2) Point the CCMUser Page to the Publisher.

Just seeing your post now. CCM version 4.0(2s)sr2a.
Regarding workarounds.
1) I am unable to even add fast dials due to the error message that comes up. There is only the "OK" softkey.
2) Already point to Publisher page
How can I determine which file it is trying to access?

most of the time, people get confusing about the PIN and the Password. If they changed their PIN, it does not update the PIN entered in the FastDial services configuration. Reset the PIN for the user, reset the PIN for fast dials and then test.

This is not the case. PIN works for other services.
