Can you copy the file from USB to flash and try it again:

Hi,
As you are trying to boot directly from a USB drive. You can try with copy .bin file in flash file and boot from the file as below:
copy usbflash0:cat3k_caa-universalk9.16.03.05b.SPA.bin flash:
boot flash:cat3k_caa-universalk9.16.03.05b.SPA.bin
You can also try to download a fresh file from Cisco.com website.

Issue is it not showing flash memory when in rom monitor mode, earlier i tried image recovery using tftp server, switch booted successfully. After booting formated flash memory and installed new image.
Everything was good but when this switch was connected to stack back and were testing on image upgrade on stack, this switch failed to boot again. Following is complete outupt.

Hi guys, im having the same issue in my 3850 .No flash: no sda9 file to enable emergency install atleast..How can i resolve?

If switch is under contract create RMA, if you got issue after upgrading software then you can remove switch from stack and try to boot using individual switch using old image on external usb drive. If switch boots up try to format flash and re-install image and try booting again. I am not sure but I think older image and newer Denali image uses different file system.

Don't panic, don't think a RMA is necessary. I had the same problem! Solved!
Try booting from the USB using an image already installed on the USB: For example :
First after inserting the USB flash, do a dir at the prompt.
Then go ahead and boot from the USB flash as listed below:
switch: boot usbflash0:usbflash0:cat3k_caa-universalk9.16.09.06.SPA.bin
You can download the image file off Cisco's website.
If this procedure doesn't work on the first trial, try it twice and even thrice. It worked for me. I think it should work for you.
