Cisco 6800IA Switch in stack failure to boot

We have a Cisco 6840 deployed with multiple 6800IA switch stacks. There is one switch stack with four 6800IA switches in the stack where switch $3 is no longer booting up. Does anyone know good troubleshooting command I can use in an attempt to diagnose why the switch may have gone down? Furthermore we are working to replace the switch, which means we will need to ensure the new switch functions as module #3 in the switch stack. Any assistance would be greatly appreciated!

I recently performed a software upgrade on several 6880 switches with attached 6800IA clients. Of the six 6880 switches two of them had 6800IA clients that did not boot up. We had to manually power cycle the IA clients to get them to boot. I was at the console of one of them and saw that it was stuck at initializing the fan controller.

We've had issues in the past with software upgrades. Apparently there is a bus on the 6800IA switches that gets in a bad state (IIC or ICC, I forget which). There is no way to know if it is in this state, but when the switch reloads you see odd symptoms. One is that that the switch doesn't boot, another is that it complains that your hardware is not authentic. In all cases doing a hard power cycle of the switch seems to be the solution. It's also a good idea to leave the power off for a little bit, I gave it 30 seconds to make sure that it was completely powered down.

Its also possible that the switch didn't get the software completely downloaded and rebooted. There was a field notice about this. It results in the switch stuck at rommon with no bootable image.  Apparently the default image distribution method deletes the existing image before it downloads the new one. So if the 6800IA rebooted for some reason as the image was downloading, it has no bootable image. In newer versions it seems that the software is downloaded before deleting the old image.

Thank you for the information! Ultimately we RMA'd the switch back to Cisco as it would not boot up after multiple attempts at powering it off and back on manually. I read through the field notice you referenced and found some good information that helped me fix another issue with a 6800ia that was caused by a bad image download. Thanks again for the feedback!
