Disregard my previous post.  I know why the switch immediately goes into ROMmon.  Look at the config-registry, it states 0x2101.  Next, look at the contents of the bootflash of the appliance:  Empty.

Config-registry value of 0x2101, particularly the last octet of "01", means "f*ck the boot variable string, boot the FIRST IOS in the bootflash".  Since there is no valid IOS file in the bootflash, the appliance goes into ROMmon.

It's a very interesting writing. i used to have same issue with my 4506-Sup6-E Switch,i was able to recover it using a TFTP server but it still goes to ROMMON mode every time it reboots. i tried changing the cofig-register to 0x2102 but it can't take any effect either.
Can you help me how to permanently fix it? here is how it looks...
