My Avaya ip phones are not acquiring an IP address from my Microsoft DHCp server, this is my first time setting up or trying to setup a voice vlan, I do have Mitel IP phones in the mix but unfortunately they are on the same vlan as my data. Usually once I configure the ports, the ports stays amber and that is all that happens.
my dhcp server is 10.200.4.64, I can ping it from my switch also.
Vlan 110 is my data
Vlan 176 is voice
The below configurations are on my core 4500 switch, I attached the initial configuration of my 3750.
Please help me out!

Hi Chad,
Your config looks ok to me. (Perhaps a bit heavy on the QOS as you have auto qos and also setting manual qos- but this should not impair DHCP in the 1st place)
You have set your interface to the phone as an access port in Vlan 176.
And your ip helper is correct.
How have you set up your test phone.
Are you selecting a voice vlan (IE tagging vlan 176)?
As you have an access port in vlan 176, your phone should NOT have vlan tagging enabled.
May be you could tell us what kind of phone it is you ar testing.
