C40 on maintenance mode

We have a C40 which is no longer booting up. it only shows the Cisco logo and will freeze. upon accessing the unit thru web, we can only see the message "maintenance mode" and the only available tab are the diagnostics and the maintenance.

We have tried to restore the unit to factory but not proceeding with the process. We are also trying to access it thru serial port but failed to connect even if we used the configured baudrate, etc.

does anyone here encounter the same issue before?

I had a similar issue a few months back with a C40, opened a case with TAC and it was RMA'd. Corrupt flash drive was the most likely cause.
