How to remove Device Associations

We have a user who moved phyical phones and extensions to a new Mac address on another phone. When I open the user's profile in CM they have the new Mac address of their new phone and the old Mac address of their old phone still listed?

How do I remove this Controlled Device under their user account profile?

Joe,

Click on the 'Device Associations' button. This will bring you to a search window where you can check or uncheck the necessary boxes to make sure the user is associated with the proper phone.
