could you help me, introduce testing evrionment ,
A SAN switch is MDS 9124.
A UCS Mini 6314
A EMC VNX 5100
on the MDS, cannot found out one of WWN of the blade server ? because of when initialization UCS profile, a blade server assigin two HBA port WWN.

Ok, to have a best practise setup (with one MDS, better of course would be 2), you have to do the following.
On MDS:
- Create 2 VSAN's e.g. 10 (Fabric A) and 20 (Fabric B); with this we simulate a logical separation of 2 fabrics; and don't forget that zoning, flogi,... is done per VSAN.
- select 2 physical FC ports eg. fc 1/1 and fc 1/2, one connecting to UCS Fabric Interconnect A, the other to UCS Fabric Interconnect B; put fc 1/1 in vsan 10, fc 1/2 in vsan 20.
on UCS
Create 2 VSAN's 10 (Fabric A) and 20 (Fabric B);
put the FC port on the Fabric Interconnect A connecting to MDS in VSAN 10, the FC port on the Fabric Interconnect B connecting to MDS in VSAN 20.
Now in the Service profile, you created 2 vhba, one connecting to fabric A resp. B; put the former in VSAN 10, the latter in VSAN 20.
Once done, show us the output of the MDS command: show flogi database
Forgot one important step: MDS has to be NPIV enabled

- topology: e.g.
do you have one or two MDS ?
is UCS FI-A connected to MDS ? is UCS FI-B connected to same or another MDS
have you created 2 different VSAN's, eg. 10 for Fabric A, 20 for Fabric B
are your vhba assigned ? vhba-1 to VSAN 10, vhba-2 to vsan 20 ?

A: Only one VSAN.

A: NO. both Vhba are same VSAN.

The reason why you have this result is: NPIV is most likely not enabled on this MDS (see my entry below)
