Hi,
I faced the error attached after loading the fnic driver and discovering the boot LUN
LUNLIST display this output

- WWPN : 50:00:d3:10:00:a5:68:4f
- WWPN : 50:00:d3:10:00:a5:68:51
- WWPN : 50:00:d3:10:00:a5:68:53
Is the Nameserver Query Response is a must to respond with 50:00:d3:10:00:a5:68:4d only ? noting that I have only boot primary path to this WWPN to have one path while windows installation but the Brocade SAN switch guy do zoning for this wwpn with all array wwpn ports.
I'll try to check today windows 2012 or ESXi on another blade to check if the error is related to win 2008

Q. Is UCS in FC end host mode ?
Q. Is Brocade in Gateway access mode ?
Q. which VSAN's are defined on UCS FI ?
Q. do you see the flogi of the link connecting UCS FI and Brocade ?
Q. is the zoning done with the initiator pwwn as seen in the SP ?

No. I can see the boot lun

no. NPIV mode

FI-A vsan 1201
FI-B vsan 1202
and we are working in FI-A path

yes. vhba login to brocade

Hythim,
You are experiencing an access failure for the LUN:
LUN ID : 0x0000000000000000 access failure
Ensure the LUN is properly masked on the storage array and the right WWPN are allowed to access it. Once this is complete, you should get serial number, vendor, model info from the array.

What type of storage subsystem do you have ? Jbod ? where each HDD (disk) has their own pwwn ? Lun masking / mapping might be wrong.
Usually (EMC, IBM,....) zoning is done single initiator - single target
