My thoughts exactly. Router debug logs show call was initiated but it does not show when call is sent back to the internal extension.
I've run out of ideas and will have to open the ticket with TAC.
I've looked in to router debugging logs and I can see call initiating but not when it's ending...

I would suggest to setup NTP server and not to adjust time ever again. On my version it can be setup under CU OS admin interface but I'm using dedicated NTP server on the network.
