What IP address does it get? One on the Data or Voice vlan? I suspect Data vlan and there is no option 150 for the data vlan.
I guess this should work but I doubt it’s a Cisco supported configuration.

What is the software that the UC560 is currently on? Go to Monitor > Telephony > Software pack to get what your software pack is and what your current IOS is.
Current software pack is 8.2.0 and the current CCA utility is 3.1(1) depending on the current IOS it may be having some compatibility issues.
If you are not on the current software please upgrade to that software pack and then retry to get everything to associate.
If that fails I recommend contacting the Cisco SBC at 866-606-1866 to better interact and get a support session going.
