UC320W outgoing calls dropped while dialing

I have this ugly issue where I have 3 pots lines from Verizon going into a UC320W and when my client makes outbound calls sometimes while dialing the phone just drops ... and the phone display goes back to the main screen

I got some logs (other than switching carriers, im out of ideas)

Today I changed it from key to pbx in a hope that selecting the line and holding to the phone would help but it appears to have not helped.

were desperate... i think im going to have to send this thing back

Stephen: Have you troubleshot the line? Meaning, if you hook up a standard phone to the line, or another system, does it work?

I'm betting that it might be a Provider issue.

Hi Stephen,

UC320W detected 200 ms CPC disconnect tone in the syslog. You may want to use an analog phone directly connected to analog phone line to see if you have the same issue. Please ask the service provider to check the phone line.
