Cisco 2802i - radio 2 has been disabled

I have an AP 2802i. I have just converted it from CAPWAP to ME (ver: 8.10.196.0).

This AP is configured to AP & Controller mode.
I created 2 SSID - 1 for 5ghz only (wlan1) and 1 for both bands (wlan2)
The wlan 1 cannot be enabled and The wlan 2 cannot broadcast 5ghz band

When I check Access Point setting, I have found that the radio 2 has gone. 
I can not enter configure terminal in CLI to turn the radio on

Can someone help to solve it?

Channel 60 & 64 (which you were trying to force) are subject to DFS (mandatory) so if the AP detected any hint of radar on these channels it would not use them.  Wider channel width increases the risk of false positive radar detection especially on lower end AP models (simpler chipset). Also remember that you have to wait for DFS scanning and detection to complete before the radio goes active, which can take a few minutes.  This is why it is best to leave the settings at auto as Flavio suggested.

 I have reset default the controller and enter BH country code.
@Dattq1201 you should be aware that in most countries it is illegal to use an AP from the incorrect regulatory domain with the wrong country settings applied because this could result in wrong channels and power settings being used for that country.  This is why it is essential to make sure you use the correct AP hardware and country configuration.

As Marce has already advised you should check your WLC config using the Config Analyzer (link below).
Note that the output you use for this tool is from the ME WLC CLI, not the client serving AP CLI.
Login to the ME WLC, enter config paging disable, then show run-config.
Save the output to a text file and then upload to the tool.

If that doesn't highlight any problems (which should be corrected first) then provide a complete "show log" from the AP CLI. Save the log to text file and attach here for us to inspect.  Also check the log on the ME WLC for any indication as to why the 5GHz is not working.

Thanks @Rich R after done I have realized that Marce was aiming to get log of show run-config in ME WLC CLI. I had done it in apciscoshell so that why I provided a wrong log.

For channel to apply 5Ghz band. Do you think radio band U-NII 1 & 3 good to use? 
I would like to use 80Mhz because almost of my devices are using wifi 6 but AP 2802i with automatic bandwithd and channel configuration is working with frequency 40Mhz so the tx/rx in uder 400mbps

It really depends on the radio conditions around your APs.
The wider the channel width then the greater the chance of interference or false radar detection.
Do you really need more than 400mbps?
Sometimes a more reliable service at slightly lower speed is preferable to an unreliable high speed service.
