Wlc interface up-down frequently

Hi everyone

I have been facing a problem in the last few days. my 2504 wlc interface up and down frequently and for this aps and clients got to disconnect. I have change the Switch port, Wlc port, and cable and the problem remains the same. Please check below for the log:

Only 1 port enabled?  What about the other ports? 

Same problem with port number WLC num 2

What model is the switch?
