I have a Cisco 4321 gateway with a PRI on it. It connects to CUCM using a SIP trunk. When I make an outbound call I can see the caller ID on the SIP leg of the call but I don't see the name going out to the telco when I run debug ISND q931. See below.

AFAICT this works as designed. The name part of the SIP dialogue would not be carried over to the ISDN side.

If that is the case how can I get the name to show up for outbound calls?

AFAIK you cannot. Not with a ISDN connection to PSTN.

That seems like a massive hole in the functionality. I know I used to do it with H323 but I don't want to have to move back to that.

The PSTN can do that on the DID.

Well I can have the Telco put a name on all outgoing calls but my issue is I have different users with different names that need to be sent out for the caller name.
