Phone A is fowarded to Phone B.
When Phone B does not answer, the call needs to get directed to Phone A's voicemailbox.
This needs to be a solution for everyone, not just individual fixes per end user.
any ideas?

Wow, is that not working already?
If phone A is forwarded to phone B, and then you call phone A, then rings phone B, if not answered it should go to phone's A Voicemail, that is how it is designed.
What is your CCM version?
What is your Unity version?
If that is not working as designed perhaps you are using Unity 5 or later and you have a call routing enabled.

This is the default behavior with CCM and Unity. Are you using these or other products. There is a setting in Unity which can change this behavior. It could have been changed with your installation. The setting is located in the Advanced Settings Tool in the Tools Depot and is called "Route by last redirecting ID".

if phone b is an external number, like a cell phone, that is where the issues arrive.

Once the call leaves the jurisdiction of the CUCM it no longer has control of that call. The caller will end up wherever the external extension sends it(for example: Cell Phone Voice mail). However, the user Cell Phones can be set up to use your Unity as their Voice mail destination, which would solve your problem.
