What exactly does mgcp fxr-package do on an MGCP gateway?
I've seen mention that it's needed when using T.38, but we're not using T.38 for faxes.

Yes, FXR package is used for T38 fax negotitation. If you are not using T38 capabilities, its not required.

The configuration for a GW that does T38 , you can use FXR as the below:-
no ccm-manager fax protocol cisco
no mgcp fax t38 inhibit
mgcp package-capability fxr-package
mgcp default-package fxr-package
mgcp fax t38 nsf 000000
no mgcp fax t38 ecm

no ccm-manager fax protocol cisco
mgcp modem passthrough voip mode nse
mgcp modem passthrough voip codec g711ulaw
