In the July 2009 Update series released for OCS 2007 R2 the mediation server can now send the calling name. See the below KB article for details.

According to my memory of an existing setup which i deployed last year.
When Cisco phone call MOC, MOC is able to see the caller name/number as MOC can do reverse number lookup. Just like the feature in your mobile phone, it will search the AD and map the name.
When MOC call cisco phone, only e.164 is shown on the IP phone.
This is normal behaviour.
However, i realize that if a MOC user is not configured with any e.164. Cisco IP phone can see the username of the MOC user when MOC user is the calling party.
Maybe there's some new update to it, i'm not very sure.

This is correct. Mediation Server in OCS does not currently pass through the calling name.
Microsoft is aware of this and we should hopefully see it fixed in a future release
