Is it possible to transfer Cisco desk phone direct calls to a cell phone? If so, what are the steps? Thanks!

No phone transfers call by self. It just sends transfer request to PBX/SBC the phone is connected to. Exact sequence depends on phone model, PBX/SBC used and it's configuration.
There's no generic answer to your question.

Will the Single Number Reach work for you? With SNR, when a call is placed to a DN the phone rings and so does a remote device such as a cell phone. The call can be picked up on either device and can be switched between them.
Here is information on setting it up. Since it's Cisco documentation the directions are overly complicated, but it's really pretty simple to set up:

Hi there
A couple of ways to do that.
1. Call forward - Forward All Softkey from phone and enter your mob number (Make sure your Forward CSS on line should have mobile access)
2. SNR - As Mahoney mentioned, here is a config part

Just note that both @Maren Mahoney and @Ratheesh Kumar assumes you have CUCM with supported phone.
Unfortunately, this thread has been originally created in Feedback Forum and moved to this community by a moderator with no knowledge of your environment.
If you have CUCM with unsupported phone (connected as generic third party SIP device) or you have no CUCM at all, answers above may not apply for your environment - so don't be confused.
