Could be an issue with the CSS assigned to forward all/busy?
Also I believe that the CSS configured for CFNA/CFB/CFA only takes effect when configured by a user (via ccmuser or the phone) - if you configure it via ccmadmin the device/line CSSs take effect, not those on the CFA/CFB/CFNA.
