We had this problem, when flash was formatted and new imaged copied to it...

Solution ..

on reload

flash_init

dir flash

boot flash:/myimagefromDIRabove.SPA.bin

then once its booted ..

conf t

boot system switch all flash1:/myimagefromDIRabove.SPA.bin

Hi

If you already have a valid image, have you checked the config register?

Just thought I'd reply to this as I was having the same issue (found this post trying to find a solution, so if others do the same in the future this may help)

I ended up running the command "emergency-install usbflash0:<binfile.bin>"

This seemed to fix the problem for us
