We had this problem, when flash was formatted and new imaged copied to it...

Solution ..

on reload

flash_init

dir flash

boot flash:/myimagefromDIRabove.SPA.bin

then once its booted ..

conf t

boot system switch all flash1:/myimagefromDIRabove.SPA.bin

Hi

If you already have a valid image, have you checked the config register?

Just thought I'd reply to this as I was having the same issue (found this post trying to find a solution, so if others do the same in the future this may help)

I ended up running the command "emergency-install usbflash0:<binfile.bin>"

This seemed to fix the problem for us

Did you solve this problem? I faced the same problem

I saw this once, and if it is on a newer 3650 then it is likely dead and will need to be swapped out (RMA'd).

Ours did it when we were trying to move a stacked switch and something must have gone wrong internally because it never booted again.
The switches were all properly grounded and still mounted when the stack cables were removed, but it shorted something out nonetheless. (I guess they aren't always as hot-swappable as they should be.)

If it's an older switch like a 3560 or 3750 then it's likely the wrong IOS, and you will need to use flash_init at ROMMON then TFTP a new correct IOS to the flash.

Just remember that you CANNOT use the up arrow in ROMMON mode to repeat the last command, anything you type (or paste) after the up arrow is always going to throw a error, even if the command isn't bad.
