Switch 3560 not giving out power

I have an issue with a 3560 in my network, after sometimes it fails to give out power to some ip phones,while others are not affected and below is the message i get

"Power given, but Power Controller does not report Power Good"

output of sh power inline and sh env all : everything seems to be ok on the switch

It's solved by a reboot of the switch, i need your assistance to know what exactly is wrong and how solve this issue once for good, i could have suspected a power environnment issue but it's the only equipement disturbing in the network.

What IOS are you running?

Can you do the following:

1.  Command:  test cable tdr int ;

2.  Wait for about 5 to 7 seconds;

3.  Command:  sh cable tdr int ; and

4.  Post the complete output please.

The strange thing is, even when the phone is directly connected to the switch it still doesn't work unless I reboot the switch

Did you tried in any other port? What was the result?

I have seen this message sometimes..It generally happens when switch boots up and all the devices request for power together...Mostly reload of the box solves the issue

and the IOS version is  Version 12.2(35)SE5

Sorry Ankur.  I will have to disagree with you on this as I see this on a daily basis.

The TDR result is very "accurate".  Pair C and D both shows signs of bad cabling.

Pair C controls PoE and Pair D controls GigabitEthernet.

The error message of "Short/Impedance Mism" is not good.  A good result to a patched outlet is "Normal". 

Another thing:  Take a look at the distance of Pairs A & B.  This figure is not THAT accurate but it does give me a worry that the cabling at the far end could be faulty or the patch port at the far-end could be faulty.

NOTE:  Whoever thought about putting a TDR into the switches deserve a raise.  You have no idea how much money we saved with this function.

To be honest with you, the Cisco documents for the TDR is very, very vague.  My experience with this feature has all been through trial-and-error.  If I publish a "blog", is there a way for the document be vetted and the original document be improved or updated?

Reloading the switch frequently is my real issue,

Have a look a this test, i connected the phone directly to the switch with a pretested cable, does the result below shows that the ports have issues ?
