We have Cisco's Speechview configured with our Unity Connection server 11.5. This is not the pro version, just the basic level. We have Unity setup to not deliver the voicemail until the transcription is received. For the most part it works well. Every now and then the users get an message that says that the transcription failed, which is understandable. Occasionally we also just get a voicemail without transcription or a failed message. The voice in the message is clear and easy to understand.
Normally this wouldn't be a big deal but the attorneys who use this make a fuss when this happens. Any ideas as to why the voicemails would skip transcription?

I experience the same problem at times. The ability of the transcription service to transcribe and transcribe accurately can depend on several factors such as:
Speechview does a pretty good job overall, but is certainly not perfect.
