Does a service-policy output have to be applied to an interface for qos to work?
here is the config but there is nothing applied to the serial interface..

Yes, the policy must be applied to an interface/sub-interface for QoS to take affect. Take a look at the QoS SRND for specific examples. Look under WAN Aggregator QoS Design -> WAN Edge Link-Specific QoS Design.

Quick question about fair-queue.
should this be set on the interface as well, as it sits now there is no fair-queue on the serial interface

The fair-queue command is not necessary on the serial interface. The command in the policy map for class class-default will fair-queue all traffic not in the 256/128KVoice class.
