Need sample script for UCCX Outbound IVR Campaign

I am looking to create an outbound IVR campaign to call patients to remind them of their appointments.  My plan was to have CCX read data from a CSV file and then start making outbound calls.  I want a prompt to play that says something like "Your appointment is Thursday at 2 PM".  

Does anybody have a sample script they can share?

Just so you know, the way OP described their desire is not an Outbound IVR Campaign. Maybe start your own thread for what it is you're trying to accomplish.
