Do you have "ccm-mananger config" and "ccm-manager config server x.x.x.x y.y.y.y" options set? Is this MGCP or SCCP? Are you sure the hostname in CUCM matches the host name of the gateway? If you have an "ip domain name" set in the gateway config, your gateway in CUCM needs to be the FQDN of the gateway. Something like "SiteA-VG450B.mydomain.com".
