Hello,
We inserted a new module with 48 rj45 ports in a 4500 with just one supervisor module. We have two 4500, and we tried the new model in the other 4500 and it´s working properly and giving a OK status. Furthermore, the module´s LED status is green. We saw that there are 8 ports (from 17 to 24) that are in faulty state, so probably this is the cause for the faulty state for the module.
We tried already a Hw reset, replace/insert again the module...
We are almost sure that it can be a Hw failure but will help if someone had the same trouble.

Verify the IOS version supports the card . If so then the card is faulty . You have an asic that is not passing POST . Ports on the cards run thru asics which covers 8 ports so there are 6 six asics on a 48 port card , one of them is not passing the POST. Get it replaced .

We are going to open a RMA, but how could be we sure if they will have to replace/repair the whole 4500? Or if just a part is having a failure?

There was no spare slot, but we remove another module (was not in use at the moment) and the card is working properly there.

We have installed in another 4500 with exactly the same IOS another card (and also this one, while troubleshooting) and it´s working properly, so it seems like it´s not a card or IOS problem.

Glenn's right. ASIC can sometimes come good. Otherwise the ASIC will deteriorate in time.

Hi Carolina,
I am experiencing same issue with one of my customer place. unfortunately they are having only 1 4507R+E chassis and 1 4548 module. how you identified this was issue with Processor Engine. however i have replaced the module 4548 as well even after getting the same error ...!! but they are having redudant X45-SUP6L-E tried by removing each at one time with no luck. pls
